APPOINTMENTS

  1. The clinic may charge you for any missed appointments unless 24 hours notice is given

  2. All consultations will be charged for and appointments must be paid for before leaving

REPEAT PRESCRIPTIONS AND SICK NOTES

  1. At least 24 hours notice must be given for repeats

  2. All patients on long term medication must have a consultation with the doctor at least once every three months

  3. You must inform us within 24 hrs of any illness if a sick note is required. Backdated sick notes WILL NOT be issued.  

  4. You may need to see the doctor before a sick note is issued.  If you have requested a sick note more than 3 times in a rolling year or there has been more than 3 months since your last consultation you MUST MAKE AN APPOINTMENT for a consultation before a sick note can be issued.   

  5. £10 per prescription/sick note must be paid upon collection.  If you wish for them to be emailed you must pay before it can be sent.

PHONE CONSULTATIONS

If you wish to speak to the doctor for any illness/concerns, this will be charged as a face to face consultation and pre-payment is required.

 

WE OPERATE A ZERO TOLERANCE POLICY AT THIS CLINIC. ABUSE OF ANY KIND WILL LEAD TO YOU BEING ASKED TO LEAVE THE CLINIC. TELEPHONE CALLS WILL BE TERMINATED.

ANY ISSUES SHOULD BE DIRECTED TO THE PRACTICE MANAGER.
